Whisper of the House — Prologue challenges ↗ describes 12 orderlies and says an orderly who flees can count. The Steam players — Challenges Not Working ↗ includes a report advising players to kill every guard, followed by a PC player saying one escape was allowed.
These accounts cannot support a blanket claim that one method always works. They may reflect different runs or versions; the available discussion does not resolve the cause.
Make the next attempt useful
For a diagnostic run, write down the objective exactly as your game displays it. Keep a separate tally for eliminated, escaped and unaccounted-for orderlies. That distinction is more informative than a total without outcomes.
